Medical Mission 2004

Dear All,

The Board of Directors of the PMMA has suggested to me that we will need a disclaimer of liability for the next mission. This is due to the size of the group.

As regards the medical liability in the Philippines, the hospital director will cover us. We need not worry about our malpractice insurance.

The place where we are going is very safe. I grew up near the area and is very close to the city hall, military headquarters and I know the people very well. We will have safety measures that we will discuss next month.

Please prepare the following documents:

download the disclaimer of liability, print, date and sign it if you agree. If you have concerns about any of the wordings, please let me know. All of us will have to sign this since the PMMA nor I will not have any form of insurance coverage for the medical mission volunteers, in case an accident happens or if we get hurt. After you have signed the form, please mail it to me.
You will need to copy your Passport, your SS card and your driver's license. You will also write your contact person in the Philippines (if there is any), your emergency contact. Place all of these info in a 5X8 clasp brown envelop. Please label your name on the envelop. You will need to seal the envelop. You will hand this envelop to me when we meet or at the airport. I will return the sealed envelops to you before you fly back to the US. If there is any special info that you want to place in the envelop in case of emergency, please include it.
If you are bringing credit cards, make sure you have a record of the credit card number and the tel number for lost cards, in case it gets lost,stolen or damaged. Make sure you inform your credit card company that you will be using your card for the 2 week period that you are in the Philippines.
If you plan on calling your family from the Philippines, purchase phone cards since it will be cheaper. Make sure that this can be used in a foreign country. If you are not able to get one, let me know in advance and I will have somebody purchase the cards fro you on the Philippines. A $20 card should be about an hour of a phone call from the Philippines to the US. Your regular cell phones will not work in the Philippines.
I am not sure about the availability of e-mails where we are staying. I can arrange with the Mayor if we need to use their internet access.
A belt bag to place you valuables during the mission.
A pen light for each individual

Please prepare all of these in advance. We are only 5 weeks away from our departure date!
